In addition, medications taken by mouth typically pass through the liver before entering systemic circulation, a process known as first-pass metabolism, which can further reduce the amount of active drug available. Because of these factors, oral administration is generally not considered an effective delivery method for PT-141.

Alternative delivery methods such as sublingual and intranasal administration are designed to bypass the liver. Sublingual formulations dissolve beneath the tongue, where the medication can be absorbed through the rich network of blood vessels in the oral mucosa. Likewise, intranasal administration delivers medication across the highly vascular nasal lining, avoiding exposure to digestive enzymes in the gastrointestinal tract.

These routes are intended to improve the likelihood that an adequate amount of the active ingredient reaches the bloodstream.
